Man Loses Arm In Chicago Workplace Accident

There has been a workplace accident in Chicago on Monday (February 23, 2015) in which a man had his arm severed by machinery as he worked at the Canton Noodle Company.

Details on this work accident are somewhat limited at this time.  However, according to the Chicago Police Department, the accident happened as the man got his arm caught in an industrial mixer at approximately 11:20 a.m. as he was attempting to make dough.

The man was taken to John H. Stroger Jr. Hospital of Cook County for treatment of accident injuries.  He was reported to be in critical condition.

Fatal “Head-On” Crash Near Gilberts, Illinois

There was a fatal Illinois “head-on” collision today (February 20, 2015) in unincorporated Kane County near Gilberts.  According to the Kane County Sheriff’s Office, the accident happened at approximately 3:15 p.m. on Illinois Route 72 about one-third of a mile east of Big Timber Road.  A five-year-old girl died from accident injuries, and three other people were hurt.

The fatal accident happened as a Mercedes C320 was heading east on Illinois Route 72.  As it came to a curve in the road, for some reason the car drove into the westbound lane and collided “head-on” with a Toyota FJ Cruiser.  The Toyota was then hit from behind by a Chevrolet.

A 5-year-old girl, Gabrielle Horvath, of Hampshire, was in a carseat in the Toyota.  She was taken to Advocate Sherman Hospital in Elgin, where she was later pronounced dead.

The driver of Toyota, James Horvath, 45 (father of Gabrielle) as well as the driver of the Mercedes, identified only as a 17-year-old boy from Carpentersville, were also taken to Advocate Sherman Hospital in Elgin.

The driver of the Chevy, David Doyle, 39, of Byron, was treated at the accident scene.

The 17-year-old boy, who lives in Carpentersville, was cited with driving in the wrong lane, police said. Police were still investigating the accident, and had not determined what caused the crash.

Fatal Workplace Accident In Joliet

Fatal Illinois workplace accidents can happen for various reasons, including improper equipment, miscommunication, poorly maintained facilities, improper procedures and various other types of negligence.

On Friday afternoon (February 16, 2015) there was a fatal workplace accident in Joliet in which one man died.

According to the Will County Coroner’s Office, the accident happened at CenterPoint Properties in Joliet, at approximately 4:00 p.m.   At that time, a worker was operating a truck to move containers, when the truck got stuck on a patch of ice.  The worker got off the truck to check the situation, and after doing so was pinned and crushed by another truck that had pulled up to help free the worker’s truck.

The worker was identified as Jeremy Hughes, 22, of Joliet.  He was taken to St. Joseph Medical Center in Joliet, where he died shortly thereafter.

An autopsy Saturday determined Hughes, who was pronounced dead at 4:54 p.m. Friday at the Presence St. Joseph Medical Center in Joliet, died of multiple injuries due to being crushed between a trailer and a semi, the coroner’s office said.

Woman Falls Into Hole In Chicago Sidewalk

Accidents that arise from people tripping and falling on sidewalks often occur when the sidewalks are poorly maintained or otherwise structurally deficient.  There have been Illinois personal injury lawsuits filed after people have been hurt in these types of accidents.   A prior post titled “Poorly Maintained Chicago Sidewalks” and the “Illinois Slip & Fall Accidents” page further discuss the issue.

Recently, there has been an incident involving a woman who was walking in Chicago when she fell into a covered hole in the sidewalk.  This accident happened near 35th and Giles.  The woman, identified as Joan Emerson, alleges that she fell into the hole and was injured, including injuries to her knee as well as fractured ribs.  The CBS Chicago article (with video) of February 13, 2015, titled “Woman Falls In Hole Along Sidewalk, Demands Accountability” discusses this accident.  According to the article, a lawsuit is being considered.

Fatal Workplace Explosion In Carol Stream, Illinois

There was a fatal workplace accident Monday night (February 2, 2015) in Carol Stream, Illinois.  A man was killed resulting from an explosion of an air tank at a manufacturing plant.

According to the DuPage County Coroner’s Office, the man who was killed has been identified as Chad Nelson, 35, of Elgin.

The fatal explosion happened at approximately 9:30 p.m. at Maac Machinery at 590 Tower Blvd.

An excerpt from the February 3, 2015 Chicago Sun-Times article titled “Man killed at Carol Stream machinery plant“:

An estimated 200-gallon air tank exploded while being tested for leaks, according to a statement from the Carol Stream Fire Protection District. There was no fire or smoke in the explosion.

Nelson, of Elgin, suffered severe traumatic injuries and was dead at the scene, according to the coroner’s office.

This workplace accident remains under investigation by the The Occupational Safety and Health Administration.
