Fatal ATV Accident In Plainfield

There was a fatal ATV accident Tuesday evening (April 19, 2016) in Plainfield.  The accident happened near Ridge Road, between 135th and Plainfield Road.

The accident resulted in the death of Alexis Richardson, 9, of Plainfield.  A man was also severely injured.

An excerpt from the April 20, 2016 Plainfield Patch article titled “Girl, 9, Dies After ATV Crash In Plainfield“:

A preliminary investigation indicates that the ATV was traveling southbound on the asphalt path along Ridge Road, continued south through the grass where it entered a culvert, lost control and crashed, according to police.

After the accident the girl was airlifted to Loyola Hospital via helicopter, but later died from her injuries.  The man taken to Rush-Copley Medical Center for treatment of his accident injuries.

The accident remains under investigation by the Plainfield Police Department Crash Reconstruction Unit.

Woman Injured After Chicago Pothole Trip & Fall Accident

Potholes and other street deformities can lead to a number of adverse consequences, including damage to cars’ tires, rims, and possibly suspension components.

Fatal Work Accident At Chicago Industrial Plant

There was a fatal Illinois workplace accident Monday morning (April 11, 2016) in Chicago’s South Deering neighborhood.

According to authorities, this accident happened in a scrap yard at Napuck Salvage of Waupaca, a scrap yard industrial plant in the 11600 block of South Burley.

At approximately 9:45 a.m., a worker got his arm trapped in a conveyor belt.

The worker was taken to Advocate Trinity Hospital, where he was pronounced dead.

According to the Chicago Fire Department, the man lost his arm during the accident and likely died from blood loss and shock.

This fatal accident remains under investigation.

Fatal Construction Accident In Des Plaines

There was a fatal construction accident today (Tuesday, April 5, 2016) on Touhy Avenue under the I-90 (Jane Addams Tollway) overpass in Des Plaines.

According to authorities, the accident happened shortly before 3:00 a.m., as a demolition company was preparing to remove a 40-ton beam from the Jane Adams Tollway.

As the workers were in lifts under the bridge, the beam fell onto a group of workers.  One of the workers, Vincente Santoyo, 47, of Berwyn, died after being taken to Lutheran General Hospital for treatment of his accident injuries.  At the time of his transfer to the hospital he was in critical condition.

Three other workers were also taken to area hospitals for treatment of their accident injuries.

As is the case in any fatal construction accident, there are many potential causes for such catastrophic accidents.  In this case, it appears that an equipment failure may have caused the accident.

OSHA said preliminary reports suggest a strap or chain that was supposed to support the beam may have failed.

The Occupational Safety and Health Administration will “conduct a thorough investigation into this tragic and preventable incident,” said Larry Joswiak, OSHA acting area director in Des Plaines. OSHA takes the stand that “nearly all worksite fatalities, injuries and illnesses are preventable,” according to its website.
