According to authorities, the accident happened shortly before 3:00 a.m., as a demolition company was preparing to remove a 40-ton beam from the Jane Adams Tollway.
As the workers were in lifts under the bridge, the beam fell onto a group of workers. One of the workers, Vincente Santoyo, 47, of Berwyn, died after being taken to Lutheran General Hospital for treatment of his accident injuries. At the time of his transfer to the hospital he was in critical condition.
Three other workers were also taken to area hospitals for treatment of their accident injuries.
As is the case in any fatal construction accident, there are many potential causes for such catastrophic accidents. In this case, it appears that an equipment failure may have caused the accident.

The Occupational Safety and Health Administration will “conduct a thorough investigation into this tragic and preventable incident,” said Larry Joswiak, OSHA acting area director in Des Plaines. OSHA takes the stand that “nearly all worksite fatalities, injuries and illnesses are preventable,” according to its website.
